Boil a large pot of salted water, cook the spaghetti according to package instructions until al dente, then drain, reserving 1/2 cup of pasta water.
Heat olive oil in a skillet, add garlic, and cook until fragrant, about 1-2 minutes.
Toss in the drained spaghetti and pour in the lemon juice and zest. Mix well to combine.
Stir in the Parmesan cheese until melted and coat the pasta, adding reserved pasta water as needed for creaminess.
Season with salt, black pepper, and red pepper flakes.
Garnish with chopped parsley if desired and serve immediately with extra Parmesan on the side.
